I am the owner of my pet leaving at f2bpetsdaycare – confirming to f2b pets day care, that I am legal owner of my dog. My dog has been given necessary vaccinations and fully vaccinated.
I understand that f2b pets day care is an open day care centre and I agree that my dog will be socialized & left with other dogs, I accept the risks involved and agree that f2b pets day care is not liable for any injuries or illness resulting during my dog’s presence at their place.
I understand that if my dog has a history of aggression or biting, f2b pets day care reserves the rights to refuse entry to my dog and I have disclosed to f2b pets day care any known dangers associated with my dog. Any behaviour deemed dangerous or inappropriate by f2b pets day care may result in the intent dismissal of my dog.
In case my pet is not picked up after a period of 10 days from the expected pickup date and there has been no communication from owner of the pet to f2b, f2b pets day care will not be liable for any consequential damages or damages beyond the replacement value of my dog.
If any medical problems develop while my dog is in the care of f2b pets day care and I cannot be contacted, I authorize them to do whatever they feel necessary action required for the safety, health and wellbeing of my dog and I agree to pay full financial responsibility for all expenses incurred.
